Joint Commission introduces campaign to reduce readmissions

The Joint Commission on December 2 launched two new resources to help healthcare providers reduce readmissions and improve the discharge process.

The resources include a new “Speak Up” campaign to “Avoid a Return Trip to the Hospital,” and a “Quick Safety” newsletter.

“Speak Up” includes an infographic, animated video, and podcast to help patients understand the steps they should take after discharge to avoid returning to the hospital.

“Quick Safety” focuses on ways to improve transitions and involve patients and their families in the process.

The materials are free and available on the joint Commission website.
