Value-based purchasing: What it means for the OR

Value-based purchasing (VBP) is part of the federal government’s plan to link payments for care to improvements in the quality of care and the patient experience. This is a major shift in how hospitals are paid for the care they provide patients. It will have a profound impact on every facility’s financial resources and will increase the importance of focusing on clinical quality and patient satisfaction measures. It is imperative that healthcare professionals are aware of these changes and what they can do to impact their facility’s scores on these criteria.
This presentation outlines the VBP program and its significance for the facility and for perioperative nurses. Ideas for addressing the perioperative measures are presented.

Developing an Effective Pain Management Program

In an environment of increasing healthcare costs, decreasing reimbursement, and uncertainty of the impact of healthcare reform, hospitals are seeking options to increase market share and develop new service lines. In perioperative services, one of the greatest challenges in maintaining an effective surgical program is the ability to sustain a financially viable anesthesiology program. One effective strategy to increase market share and off-set the costs of an anesthesiology payment system is the development of an effective chronic pain management program.

Jerry Ippolito will discuss key elements required to develop a financially successful pain management program and describe necessary resources and operational efficiencies.

Leader: Jerry Ippolito, MBA, MHSA, President, OR Efficiencies, LLC.

The Older Adult Surgery Patient: Preop to Discharge

The US’s older adult population, those 65 and older, is approximately 40 million and expected to nearly double by 2030. Due to improved living conditions and advancement in medicine and technology, the fastest growing cohort is 85 years and older. These patients are arriving in our ORs for elective surgery, urgent/emergent conditions and trauma. Many OR nurses have never had geriatric nursing courses yet they are caring for these patients who bring unique issues that need to be addressed from preoperative assessment through discharge from PACU. The speaker, an experienced OR nurse and gerontologist, will discuss the challenges this population brings, including physiological change, medication management, sensory deficits, discharge planning, and more.

Leader: Judy Dahle, MS, MSG, RN, Education Coordinator, OR Manager, Inc, and Gerontologist.

Redesigning Surgery Scheduling Workflow

Is your scheduling process subject to lack of standardization, inaccuracies, and inefficiencies? The surgery scheduling workflow process at Northwestern Memorial Hospital (NMH) had many of these symptoms. Patients, surgeon's schedulers, and surgeons were dissatisfied with the system. Inaccurate information was being entered when cases were reserved by phone or email.

A multidisciplinary team used principles of LEAN to evaluate the current workflow process, which fell under the three categories: reservations, confirmations and schedule production.
Based on their well-received presentation at the recent Managing Today’s OR Suite conference, the speakers will share how they redesigned surgery scheduling workflow, resulting in decrease in phone call abandonment rate, error rate, scheduling delays, and productivity improvement. Learn how a value stream map can reduce or eliminate nonvalue-added functions in the workflow and help streamline operations.

Leaders: Arshia Wajid, MBA, MPH, Project Manager, Surgical Services, Northwestern Memorial Hospital, Chicago

More Cases in Less Time – Your Key to OR Optimization

Learning how to use empirical data to see how shifts in case volume affect staff downtime and OR idle time. With use of data based on experience and observation, you will see dramatic results in terms of:

• fewer gaps in the schedule
• reduced number of after-hours cases
• higher incidence of on-time first case starts
• streamlined turnaround time
• increased physician satisfaction and engagement
• lower staffing costs.

Analytics for optimization depict what happens on the schedule, pinpointing opportunities for demand match staffing and for eliminating causes of waste. The analytics also function as a driver of consensus among surgeons, OR administration, anesthesiologists, and C-suite executives as well as a catalyst for governance best practice. During this webinar, the speakers will share how this process helps hospitals anticipate health care reform by focusing on accountability, quality, and collaboration while considering fewer resources to pay for care. They will share case studies to demonstrate how metrics were used to attain desired outcomes. They will discuss a road map to optimize your OR performance.

Leaders:
Starla M. Wilkes, BSN, RN, CNOR, Vice President – Account Services, Syús

Getting Ready for Value-Based Purchasing

What do you need to do to prepare for value-based purchasing? Under the he Patient Protection and Affordable Care Act (PPACA), provider organizations are encouraged to manage patient populations cost effectively without compromising quality of care.

A key goal is for providers to work together to manage clinical processes while accepting risk for savings and attainment of quality of care thresholds. The Hospital Value-Based Purchasing Program (VBP) represents a fundamental shift in how providers will be paid, requiring important changes in operating models, clinical processes, and alignment of interests between physicians and hospitals.

With perioperative services experience as a hands-on clinician, hospital administrator, and consultant, this webinar leader will provide essential information about VBP, including what hospitals should be doing now to prepare for mandated value-based payment.

Vendor Management in the OR

Traditionally, vendor representatives have cultivated long-standing close relationships with physicians, sometimes dating back to medical school. Some physicians have developed loyalty to vendor representatives that trumps loyalty to provider organizations. These well-established relationships may drive higher device costs for organizations.

So how can health care providers better manage vendors? One answer is the implementation of a strictly enforced, robust vendor access and credentialing policy. There are automated systems available that register and provide credentialing guidelines and background checks to ensure that vendors are good partners.

This webinar  looks at vendor management from a corporate, end-user, and vendor perspective to guide your implementation of an effective vendor management program.

The bad news is that it can also put a facility at risk if the most current and appropriate sterilization disinfection practices are not being followed.

You will hear the latest trends and information related to instrument processing. You will be able to identify practices in your facility that may need to be revisited.

Invite your SPD personnel to listen to this outstanding expert in the field of sterile processing and infection control. This webinar will provide an excellent opportunity for OR and SPD personnel to learn together.

This is a one-hour webinar.

Silence Still Kills: Opting for the Crucial Conversation

Health care professionals are often reluctant to speak up when they observe colleagues doing something wrong. In the 2005 study, Silence Kills, VitalSmarts and the AACN found that 84 percent of healthcare professionals observe colleagues take dangerous shortcuts when working with patients and yet less than 10 percent speak up about their concerns.

Since that time, the healthcare community has turned to safety tools and checklists to reduce unintentional slips and errors. And yet, a new study called The Silent Treatment has found that the effectiveness of safety tools is undercut by undiscussables. Every day, health care professionals are making calculated decisions to not speak up—even when safety tools alert them to potential harm.

The Silent Treatment reveals that despite the safety interventions taken in the last decade, silence still kills. Safety tools do not compensate for crucial conversations failures in the hospital.

Join David Maxfield, a bestselling business author in leadership and influence, as he shares with us how to apply the ground-breaking implications from The Silent Treatment study. Learn about the conditions that make a conversation crucial, what it means to toggle between silence and violence in a crucial conversation and how doing so leads to failure.

Specialty Teams in the OR

There are many definitions for what OR specialty teams really are. It may be staff who consistently work in the same service or for the same physician but don’t take call for that service or physician. It may be an exclusive team that only works for that service or physician and takes call exclusively for that specialty. Is a team a full complement of staff who work in the OR or just a specific few who maintain expertise in particularly complex cases in that specialty? Do teams receive special education, schedules, or compensation? More importantly, by what metrics are these teams evaluated in terms of quality, efficiency, cost, and benefit? During this webinar, we will explore these questions and discuss what works, what doesn’t, and how to tell if specialty teams are working or will work for you.
